Description
At CLEAR, we are dedicated to enhancing the lives of individuals facing mental health challenges in the Kirklees area. Our mission is to provide flexible, creative, and responsive support that empowers our clients to lead fulfilling lives.
The post holder will be responsible for developing and contributing to a flexible,
creative and responsive support package for individuals with mental health issues in
the Kirklees area.
Support is provided through the use of group and 1:1 support with an emphasis on co
production. The post holder will build and maintain positive relationships with service
users and Kirklees statutory and voluntary services and work alongside the wider
Clear team to ensure effective service delivery. They will undertake assignment work
to achieve a Level 3 Qualification that will enhance their learning and knowledge as
an Apprentice Support Worker.
MAIN TASKS:
- Assist the team of Support Workers in the delivery of groups and courses within
service.
- Be involved in processing referrals, assessing potential clients and dealing with
general enquiries to the service.
- Ensure client records and other information systems are completed accurately
and within agreed timescales.
- Produce letters and other written communication to a high standard.
- Undertake data collection and administration, ensuring this is completed to a
high standard and on time to enable accurate monitoring in line with
commissioning requirements.
- Learn the importance of Support Planning with Clients and contribute to Clients
Support Plans and goal setting.
- To learn and work within agreed risk management plans in line with therapeutic
risk taking and Community Links Policy.
- Acknowledge and work flexibly with carers whilst respecting the overall wishes
of the client.
- Adhere to service targets around utilisation and key performance indicators.
- To creatively contribute to the achievement of annually set individual and team
targets.
- To participate in regular supervision and attend regular meetings with the Work
Tutor.
- Keep up to date with policies, guidelines, procedures and practices, including
those in relation to risk and personal safety.

- Good verbal and written communication skills and numeracy skills sufficient to be able to make accurate reports and written records.

You will be working towards the Level 3 Apprentice Support Worker Which includes:
•Knowledge, Skills and Behaviours
•Level 3 Diploma in Support work
•Functional skills in Maths and English if required
